What is All On Four Dental Implant?

All On Four dental implant is a procedure that allows the fixation of dental prosthesis on four implants placed at certain angles in completely edentulous patients on the same day.The All on four provides a surgical treatment option for replacing a full set of teeth in the upper or lower (or both) jaw with just four dental titanium implants  placed precisely at strategic positions and at certain angle. This will serves as a solid foundation to support a fixed full bridge (anchored by four implants to the jaw) that is natural looking, stable, durable and stays permanently fixed in place providing the functionality and bite quality that you would expect from natural teeth. If they do not have sufficient bone structure, sinus lifting surgery or graft addition procedures are performed in Golden Smile polyclinics.

What are the stages of All On Four treatment?

Patients who are planned to have All On Four dental implant treatment must first undergo a detailed clinical and radiological examination at any branch of Golden Smile polyclinics. Measurements are made on computerized tomography (CT) and a plan suitable for the patient is made.
All On Four procedure consists of three stages:

Step1: Consultation

When we recieve your Xray and teeth pictures over whatsapp then first consultation will start. Our dedicated dentists who have more then 25 years experience on surgery on cosmetic dentistry will look into your files and you will be asked some additional questions in order to understand what exactly you needed. X ray is very important to see your bone and make desicion on All on 4 process or we may go with normal dental implant process.

Step2: Implant Placement

All on 4 procedure is composed of two parts as surgery and dental prothesis process. All on 4 process is a procedure that can be easily applied under local anesthesia. But it can be realized with sedation or under general anesthesia for patients with anxiety and high level of fear.

Step3: Recovery

After your dental implants have been placed, the final step is to allow them to heal properly. Recovering from an implant surgery would not be painful if you take your antibiotic and painkillers which will be given by our experienced surgery and cosmetic dentists.
Soon after the surgery, you may experience gum bleeding. A small amount of bleeding is normal, it will decrease in 1-2 days. The surgical site may swell up to 2-3 days. The dentist would prescribe medication to manage the swelling. In some cases, the surgical site first turns black, then turns yellow and will heal after a while.

Healing periods usually takes between 2-5 months, called osseointegration, in which new bone formation occurs around the implants placed. Osseointegration happens when bone cells attach themselves directly to the implant’s surface. It plays a vital role in the success of dental implants. To allow this to happen, your dentist will place you on a temporary soft foods diet to avoid placing too much force on your newly placed implants. If too much force is exerted on your implants too soon, this can cause them to become loose or to heal incorrectly. Only after osseointegration has occurred is it possible for your dentist to start work on your permanent dentures.

Overall, the dental implant process may take 15 days or 3 months from beginning to the end. A large part of this time is left for healing and letting new bone grow in your jaw. In some situations, certain steps can be combined – depending on the specific procedure performed or the materials used.

1.When Bone Grafting is Required

If your jawbone is too soft or not thick enough, a bone grafting procedure may be required before you can have the implant placed into the jaw. When you chew, a strong pressure is exerted on your bone, and if it isn’t able to support the implant, the surgery is likely to fail. A bone graft can be used to create a more solid base for the implant.

There are a few bone grafting materials that can be used for this purpose:
  • Natural bone graft: for example, bone from another location in your body
  • Synthetic bone graft: a bone-substitute material

You may have to wait several months for the bone graft to heal enough to support a dental implant. Sometimes, you may need only minimal bone grafting, which can be completed at the same time the tooth is extracted or during the implant surgery.

The condition of your bone will largely determine the specifics of the entire dental implant process for you. It may take up to a few months for the bone that has been transplanted to grow enough amounts of the new bone to support an implant. 2. Placing the Dental Implant

During the oral surgery to place the implant post, your oral surgeon in Golden Smile makes a small cut in order to open the gum and reveal the bone. Then, the implant dentist drills holes into the bone, where the metal post will be later placed. The post is implanted deep into the bone, as it will serve as the tooth root.
At this stage, you will still have a gap in place of your missing tooth. If needed, a type of temporary partial denture can be placed for appearance(recommend to not have temporary for better healing). It can be removed for cleaning or while you sleep.

3. Waiting for Bone Growth

After the metal post has been placed in your bone, the process of osseointegration begins. This means that the jaw bone unites with the dental implant surface. ➢ Immediate Post-surgery (24-72 hours after surgery): Initial healing is characterized by the formation of a blood clot at the surgery site. This can cause swelling, bruising, and discomfort. Pain relievers can be used to manage post-surgery pain. Some painkillers can have side effects and aren’t recommended for people with certain health problems. When prescribing pain medications to our patients, we will ask if any health issues they have.

➢ First Week: The initial healing phase continues. Swelling and discomfort gradually decrease over the first week after surgery. Patients must eat soft foods and avoid disturbing the surgery site.

➢ 2-6 Weeks: Soft tissue around the implant site starts healing. Patients should maintain oral hygiene during this period to prevent infection.

➢ 6 Weeks to 3 Months: The implant starts to bond with the jawbone. This marks the beginning of osseointegration. During this period, patients should see their dental surgeons regularly for checkups. Regular checkups give surgeons the opportunity to monitor progress.

➢ 3-6 Months: Osseointegration continues during this period. For some people, this process may be quicker. As a result, the next phase of the treatment (which may involve placing the abutment and crown) may occur sooner. For people who are experiencing bone loss in their teeth or individuals who need bone grafts, the process may take longer.

➢ 6 Months and Beyond: Once osseointegration is complete, the abutment and crown can be placed.

4. Placing the Abutment

When the bone growth process is complete, you may require another surgery to place the abutment, or the part where the crown will attach in the future. This minor oral surgery is usually takes very short time and done with local anesthesia.

However, dental bridges require neighboring teeth to be filed down since these teeth are used as the supporting pillars for the bridge, otherwise roots are moving neighbouring and another risk to loose teeth which are good. Porselen Kaplama ile Zirkonyum Kaplama Arasında ki Farklar?

Porselen kaplama, altta bulunan metalin görüntüsünü yansıttığı için donuk ve mat bir görüntü verir. Alttaki metal, zamanla okside olduğu için, diş etinde de renklenmeye sebep olur. Zirkonyum kaplama ise, diş etiyle de uyumlu (Biyouyumlu) bir malzemedir ve çok uzun süre dayanıklı olduğu gibi hiç bir zaman deforme olmaz. Implant yapılan yerler arka bölgelerde ve ağız yapısına uygun olarak görüntülenmiyorsa daha sağlam bir yapı oluşturması için porselen kaplama seçilebilse de ön dişlerde estetik olması açısından mutlaka zirkonyum seçilmelidir.
